4、《外國人就業(yè)許可證書》（正本）。
Your employment license.(Original and copy).
5、《勞動(dòng)合同》原件及復(fù)印件。
Your Labor contract.(Original and copy).
6、護(hù)照及職業(yè)簽證的原件和復(fù)印件。
Your passport and your professional vi
sa(Z type of visa).
7、二寸照片3張。
Three pieces of 2 inches photos.
